I-693 Immigration physicals by Civil Surgeon

  • Normally the I-693 form is submitted with the I-485 form.  Staying Healthy offers 16 USCIS designated clinics for Immigration Vaccines and Tests.   We expertly handle more immigration applicants than anyone else and will get it done right for you.  We can also provide limited help with the I-485 form for those wishing to complete the form themselves or online and we do provide  referrals to reputable Immigration Lawyers who refer to us.  We offer service in English and Spanish at competitive prices.  The patient will see the civil surgeon after all tests and vaccines are completed.    Please note that if you give us your number, you are giving us permission to SMS text you for appointment confirmations.  Final exams are performed by one of  our  on-staff physicians:  Dr. Sohail Saeed, Dr. Nathan Wright or Dr. Igor Shkolnik.

The I-693 form must be completed for anyone applying for an adjustment of status or  Green Card (I-485).

 Once you make an appointment, we will provide detailed instructions on what to expect.  Generally, a blood draw and urine test is necessary. The specific tests will be based on the patient’s age and personal medical history and other health factors. Following the first visit, the applicant will be seen in clinic for a second follow-up visit. The entire process may take up to 14 days.


Instructions for your immigration physical:

You must bring all medical records including vaccinations and your Passport/VISA. Have them translated into English or bring a translator with you. **It is the responsibility of the applicant to have any record used for the Immigration Physical to be officially translated into English.

The following is a list of items you are required to bring to your examination:

  1. Your personal photo identification with a photo such as driver's license or Passport.
  2. I-693 form 
  3. Vaccination Records

At Passport Health DFW, we stock all required vaccinations which can be given on the first or second visit.

Once the I-693 is completed, the original is provided in a sealed envelope for delivery to USCIS and a second copy is provided for your personal records.

If you are applying for an adjustment of status to become a lawful permanent resident, normally form I-485,  use the I-693 form.

I-693 Immigration Form

Form I-693 reports results of a medical examination to U.S. Citizenship and Immigration Services (USCIS).   

  1. The medical report form is to be completed in English, typed, dated, and signed by the civil surgeon.
  2. The results of required tests for tuberculosis and syphilis must be entered in the appropriate spaces on the medical report form.

 You should schedule the medical examination as close as possible to the time you file for adjustment of status, respond to a Request for Evidence, or attend an interview (if applicable).  NOTE: The Form I-693 must be dated no earlier than 60 days before you filed your underlying application under normal circumstances

Under the immigration laws of the United States, a foreign national who applies for an immigrant visa abroad, or who seeks to adjust status to a permanent resident while in the United States, is required to receive vaccinations to prevent the following diseases:

  • Mumps
  • Measles
  • Rubella
  • Polio
  • Tetanus and Diphtheria Toxoids
  • Pertussis
  • Haemophilus influenzae type B
  • Hepatitis B
  • Any other vaccine-preventable diseases recommended by the Advisory Committee for Immunization Practices

The I-693 Medical Review

  The medical examination requires a medical history, obtained by the civil surgeon or a member of the physician’s professional staff, from the applicant (preferably) or a family member, which includes:

  1. a review of all hospitalizations
  2. a review of all institutionalizations for chronic conditions (physical or mental)
  3. a review of all illnesses or disabilities resulting in a substantial departure from a normal state of well-being or level of functioning
  4. specific questions about psychoactive drug and alcohol use, history of harmful behavior, and history of psychiatric illness not documented in the medical records reviewed and
  5. a review of chest radiographs and treatment records if the alien has a history suggestive of tuberculosis

Civil Surgeon Physical Exam


 The physical examination is to include:

  1. a mental status examination that includes, at a minimum, assessment of intelligence, thought, cognition (comprehension), judgment, affect (and mood), and behavior
  2. a physical examination that includes, at a minimum, examination of the eyes, ears, nose and throat, extremities, heart, lungs, abdomen, lymph nodes, and skin
  3. all diagnostic tests required to identify communicable diseases of public health significance, as well as other tests identified as necessary to confirm a suspected diagnosis of any other Class A or Class B condition
